How to apply...
Complete the enrollment and inclusion forms, submit the registration fee and the advance last month's tuition payment to hold your child’s position. The advance payment is credited to the last month of the school year (May 2022) and is non refundable. The health form is due before the first day of school with early enrollment or within 30 days of the first day of regitration for those who register after the school year begins.
Scholarships are available to qualifying families.

Print and complete along with a copy of the child's insurance card-
Print, read and sign-
Print and complete top portion, have child's doctor complete, sign and date bottom portion.
Supply a copy of child's immunizations.
Every child must have a current health certificate each year of attendance at CCP.
