Registration

How to apply...

Complete and submit the enrollment forms, registration fee, and advance month's payment.

The health form and immunization records may be turned in after the registration paperwork, but must be turned in by the first day of school.

If enrollment occurs after the beginning of school, the health form can be turned in within the first 30 days of attendance.

The advance payment is credited to the last month of the school year (May).
The registration fee and advance payment are nonrefundable.
Registrations are accepted in the order they are received until classes are full.
When classes become full, a waitlist will be started. There is no charge to be placed on a waitlist. Families on the wait list will be contacted when a class position becomes available.

Print, complete, and submit with the registration fee and advance month payment.

Print and have the child's health provider complete, sign, and date the health certificate.
The completed form is due before the first day of school or within 30 days of enrollment, whichever occurs first. The health certificate may be turned in after the registration paperwork and fees are submitted.
A copy of the child's immunizations is required.
